Fidelity check acknowledgment

We received the attached check donation where it's made out to us c/o donor name. The donor included a note saying "please send the receipt to my address". My question is, does his acknowledgment get tax verbiage? Or is this technically from Fidelity? The EFTs are more straight forward, but I've never gotten a check from Fidelity. Thank you

This check came from a brokerage account - not a DAF.  Therefore, the acknowledgment does need to include the necessary goods and services language.  The receipt goes to the donor and not Fidelity.  Fidelity is acting as an agent for the individual.
